When an application to add father's details is received, the Assistant Registrar - Registrations will normally recall the original birth registration statement from Archives. This is to verify details that may have been provided at the time of birth and also to find out whether reasons were given at the time of registration as to why father's details should not be included.
Once the statement is received from Archives, the Assistant Registrar - Registrations will review and assess the application in consultation with the Registrar. Following this, the application will either be approved and a new birth certificate issued with the father's details added, or you will be contacted with a request to supply further evidence or information.
Please note: changing a child's name is a separate process. Please refer to our section on Changing a Child's Name for further information.
To add father's details to a birth record:
- Complete an Adding Father's Details to a Birth Record statutory declaration form.
- Ensure the form is signed by both natural parents and witnessed by a Justice of the Peace or a Solicitor.
- Lodge the form with the Assistant Registrar - Registrations in person or by mail.
- The form must be lodged with:
- Child's Birth Certificate (original NOT a photocopy)
- One (1) document of identification for each parent. Photocopies must be certified true and correct by a Justice of the Peace or a Solicitor.
The following documents are acceptable as identification:
- Passport
- Driver's Licence
- Taxation Certificate
- Citizenship Papers
- Defence Force or Police Photo ID
- Social Security Benefits Card
