Alternatively, you may write a letter to our office requesting the certificate(s). If you are applying for another person's certificate, please refer to our Access Policy first to ensure you are able to access the information you need. In your letter, please include the following information:
- The type of certificate you are requesting (eg birth, death or marriage)
- Who the certificate subject is (eg yourself, your mother/father, your son/daughter etc)
- The date of the event (eg date of birth, date of death or date of marriage)
- The full names of the parents
- The place where the event occurred (eg North Adelaide, Parkside etc)
- A Postal address if you would like the certificate posted, or a Residential address if you would prefer to collect the certificate.
How to submit your application
By Post- Correctly complete the application form
- Disclose credit card details and expiry date on the application form, or alternatively attach a money order or cheque made payable to Births, Deaths and Marriages.
- Attach your photocopied identification documents
- Post your application to: PO Box 1351, Adelaide SA 5001
In Person
Come into our office and complete the appropriate application form, present your identification and provide payment.
Please note: you can also submit an application through our Online Certificate Application facility (credit card payments only).
